## Executive GIST (TL;DR)
- This systematic review and case report examines the rare phenomenon of **spontaneous complete regression** of congenital brain tumors, combining an institutional posterior fossa case with a literature synthesis. - The authors followed PRISMA methodology and registered their protocol with INPLASY; searches covered Web of Science, Scopus, PubMed (MEDLINE), and Embase through 5 April 2026. - Six eligible published case reports were identified and, with the institutional case, yield data on seven patients in total. - Clinical presentation was heterogeneous: signs of intracranial hypertension in 4/7, neurological deficits in 2/7, and no neurological symptoms in one neonate. - Tumor locations varied: deep brain structures in 3/7, cortical regions in 2/7, and extracerebral sites in 1/7. - Histopathology spanned grades: high-grade tumors in 3/7 and low-grade tumors in 2/7; details for the remaining cases were not fully reported in the abstract. - Reasons for avoiding immediate surgical resection included risky tumor topography (2/7), a presumption of benign biology with a watch-and-wait approach (1/7), and parental decision for hospice care (1/7); in three patients surgery had been planned pending weight gain. - Complete spontaneous regression occurred between 3 and 33 months of age across the cases. - The report emphasizes the rarity and incomplete understanding of this phenomenon, and highlights heterogeneity in histopathology, lesion localization, and often subtle clinical presentation in neonates and infants. - The institutional case involved a posterior fossa tumor that involuted by age 3 months; additional case-level details (imaging, exact histology, and management timelines) were not reported in the abstract.

The study protocol was registered with INPLASY. A systematic review of the articles published from inception to 5 April 2026 in Web of Science, Scopus, PubMed (MEDLINE), and Embase was conducted. Six eligible case reports were identified and included in the analysis. **Results:** Initial clinical presentation varied, with neurological deficits observed in 2/7 patients, signs of intracranial hypertension in 4/7 patients, and no neurological symptoms in one neonate. Tumors were most commonly located within deep brain structures (3/7), followed by cortical regions (2/7) and extracerebral locations (1/7). Histopathological grading revealed high-grade tumors in 3/7 patients and low-grade tumors in 2/7 individuals. Tumor resection was not pursued because of risky tumor topography (2/7), presumed benign tumor biology with a wait-and-see strategy (1/7), or parental decision to pursue hospice care (1/7). In three individuals, tumor surgery had been initially planned after gaining a greater body weight. Complete spontaneous regression was achieved between 3 and 33 months of age. **Conclusion:** Spontaneous complete regression of congenital brain tumors is a rare and incompletely understood phenomenon. Our study highlights the heterogeneity of histopathological findings, lesion localization, and the subtle clinical presentation in this age group. **Keywords:** Posterior fossa tumor; Prenatal brain tumors; Solitary fibrous tumor; Tumor involution. © 2026.
