# Case fatality in children treated in pediatric vs adult ICUs in Germany: nationwide cohort analysis

## Executive GIST (TL;DR)
- Nationwide retrospective cohort of children aged ≥28 days and <18 years receiving **complex intensive care treatment (CICT)** in Germany using hospital discharge data from 2016–2023. Total cases analyzed: 143,034. - Cases were classified by treating department as **PICU** (67.8%), **AICU** (14.0%), or interdisciplinary (18.2%). - Crude odds ratio (OR) for in-hospital death comparing PICU versus AICU was 1.14 (95% CI 1.03–1.26), but this association reversed after adjustment. - After adjustment for age, acute organ dysfunction, and chronic conditions and accounting for hospital as a random effect, the adjusted OR for death in PICUs versus AICUs was 0.73 (95% CI 0.63–0.84), indicating lower risk in PICUs. - Combining PICU and interdisciplinary cases versus AICU produced a fully adjusted OR of 0.61 (95% CI 0.54–0.70). - Estimated excess deaths across the study period associated with AICU care were 100; this estimate rose to 191 when interdisciplinary cases were counted as pediatric. - A complementary qualitative survey of pediatric and adult intensivists identified three main domains for AICU admissions: capacity constraints, organizational factors, and clinical expertise. - Authors conclude that children treated outside routine pediatric intensive care settings had higher risk-adjusted case fatality and that findings support treating critically ill children in settings with routine **pediatric intensive care** experience. - Ethics and data: Secondary fully anonymized data from the Federal Statistical Office were used; no ethics approval was required. We assessed case fatality of children receiving complex intensive care treatment (CICT) by treating department in Germany and explored reasons for admission to adult intensive care units (AICU). Methods: Retrospective study using the German nationwide hospital discharge dataset 2016 to 2023.
