## Executive GIST (TL;DR)
- Investigators sequenced fetal **cell-free DNA** from discarded cultured chorionic villus biopsy media (n=30) to build an early prenatal methylome reference. - The sequenced fetal cfDNA samples had a reported median purity of 94.7% and the assembly covered approximately 26 million CpG sites. - Median sequencing depth for the assembled reference was 207x, enabling high-resolution methylation calls across the first trimester genome. - The study team developed an analytical enrichment model that identified millions of CpG markers and assigned predicted enrichment scores for fetal-specific hypomethylation. - Guided by that model, the authors adapted an assay named **epiSIFT** as a genome-wide method selective for unmethylated DNA molecules. - In 28 prenatal plasma cases, use of **epiSIFT** on the top one-eighth of markers increased the median **fetal fraction** from 5.1% to 28.4%, a reported 5.6-fold gain. - The authors report the work establishes a **first-trimester methylome** reference and demonstrates epigenetic enrichment of the circulating fetal genome. - Competing interests disclosed include provisional patent filings related to **epiSIFT** and FLEXseq by named authors. - The report is a preprint and has not undergone peer review; the abstract does not include full experimental or demographic details, which are available only in the full manuscript and supplementary material.

However, low fetal fraction limits current testing. Fetal cfDNA can be enriched via hypomethylation, but the informative sites require a first-trimester fetal methylome reference. Here we sequenced fetal cfDNA (median purity 94.7%) derived from discarded media from cultured chorionic villus biopsies (n=30) and assembled a first-trimester methylome across 26M CpG methylation sites at a median depth of 207x. We then built an analytical enrichment model that identified millions of CpG markers and assigned predicted enrichment scores. Guided by this model, we adapted epiSIFT as a genome-wide assay selective for unmethylated DNA molecules. In 28 prenatal plasma cases, epiSIFT raised the median fetal fraction from 5.1% to 28.4% (5.6-fold gain) using the top eighth of markers. These findings establish a first-trimester fetal cfDNA methylome reference and demonstrate epigenetic enrichment of the fetal genome. ### Competing Interest Statement T.L. and W.G. have filed a provisional patent related to epiSIFT, application number PCT/US26/20443.
