## Executive GIST (TL;DR)
- The TREOCAPA trial evaluated prophylactic **paracetamol/acetaminophen** given during the first 5 days of life to infants born before 29 weeks' gestation to assess survival to 36 weeks' post‑menstrual age without severe neonatal morbidity. The original multicentre, pan‑European phase II/III trial did not include long‑term follow‑up at outset due to time constraints. - TREOCAPA‑LT is a follow‑up study initiated after the trial started to assess outcomes at 2 years corrected age for participants from centres in countries participating in the follow‑up. Eligible children are those enrolled in the TREOCAPA phase III trial who are aged 23.5–27.5 months corrected age during the study window. - Families are contacted by clinical teams and invited to complete an internet‑based parent questionnaire. The primary outcome is cognition measured by the non‑verbal Cognitive Scale standard score from the **PARCA‑R** (Parent Report of Children’s Abilities‑Revised) questionnaire. - Secondary outcomes include rates of moderate to severe neurodevelopmental impairment, health service use, and associated costs. Analyses will be by original randomisation group from the TREOCAPA RCT. - Ethical approval was obtained first in France (sponsor) and subsequently in all participating countries; the French approval body named is Comité de protection des personnes Est I of Centre hospitalier la Chartreuse, Dijon. The study will publish results open access per funder regulations. - Trial registration number is NCT06064825. The funder is Inserm (MESSIDORE 2022). No new participant data are presented in this protocol manuscript; the Data Availability statement reports not applicable. - Competing interests: one author (SJ) is part of the team that developed the **PARCA‑R** tool (professional, non‑financial interest). Multiple authors are affiliated with INSERM; the funder had no role in protocol development or writing despite affiliations. Other authors declare no competing interests. - The manuscript confirms compliance with ethics guidelines, informed consent, trial registration, and use of relevant reporting checklists. Details of statistical methods are described as part of the protocol and analysis plan in the source document.

However, due to time restrictions, long-term follow-up was not planned as part of the initial trial despite recommendations for neonatal trials to include endpoints measuring efficacy after discharge from the neonatal unit and on-going debate about the long-term effects of paracetamol in the neonatal period. This protocol describes a study to follow-up participants in the TREOCAPA trial at 2 years corrected age initiated after the trial's onset. Methods and analysis: The TREOCAPA Long Term (TREOCAPA-LT) study population consists of all children included in the TREOCAPA phase III trial in centres in countries participating in the follow-up study who are aged between 23.5 and 27.5 months corrected age during the study period. Families are contacted by the clinical teams and a parent or guardian is invited to complete an internet-based (online) questionnaire. The primary outcome is cognition, as measured by the non-verbal Cognitive Scale standard score from the Parent Report of Children's Abilities-Revised (PARCA-R) questionnaire. Secondary outcomes are moderate to severe neurodevelopment, health service use and costs. Data will be analysed according to the study groups to which children were randomised in the original TREOCAPA RCT. Ethics and Dissemination: The TREOCAPA-LT protocol was first approved by ethics' committees in France (the study's sponsor) and subsequently by all participating countries. Results from TREOCAPA-LT will be published in open access journals in accordance with the funder's regulations. Registration: https://clinicaltrials.gov/study/ NCT06064825 Competing Interest Statement SJ is a member of the research team that developed the PARCA-R questionnaire and the standardised scoring system used in this study; this is a professional, non-financial interest. ASM, JCR, MU, CA, VD, MD, MH, MM and JZ are all affiliated professionally with INSERM; the funder had no involvement with the development of the study or the writing of the protocol despite author affiliations with the funder. All other authors declare no competing interests. Clinical Trial NCT06064825 Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
