## Executive GIST (TL;DR)
- Bronchiectasis has historically been considered a predominantly **neutrophil-driven** airway disease, but a substantial subset of patients show **eosinophilic** inflammation, prompting reconsideration of disease endotypes. - Eosinophilic bronchiectasis is identified using **blood** or **sputum eosinophil** counts; however, the optimal biomarkers and diagnostic thresholds are not standardized in the literature. - Eosinophilic inflammation is observed in bronchiectasis and in overlapping **type 2** airway diseases such as **asthma**, **allergic bronchopulmonary aspergillosis (ABPA)**, and **cystic fibrosis**, complicating diagnosis and interpretation. - The presence of eosinophilia may inform disease severity, prognosis, and therapeutic selection, suggesting a potential role for personalized management based on inflammatory endotype. - Current evidence for targeted therapies in eosinophilic bronchiectasis is limited and primarily observational; data suggest patients with elevated eosinophil counts may respond better to **inhaled corticosteroids**. - Early studies of **biologic** therapies show promise, particularly for patients with coexisting ABPA or asthma, but their efficacy in eosinophilic bronchiectasis independent of overlapping type 2 conditions remains uncertain. - Future research priorities include establishing clinically relevant biomarkers to standardize the definition of eosinophilic bronchiectasis, clarifying its relationship with other type 2 airway diseases, and defining indications for **inhaled corticosteroids** and **biologics** in this endotype. - The concept of eosinophilic bronchiectasis supports a move toward precision medicine in bronchiectasis care, but current recommendations are constrained by limited and largely non-randomized evidence.

Recognition of eosinophilic inflammation in a substantial subset of patients has challenged this paradigm. This review summarizes emerging evidence regarding its pathogenesis, clinical significance, and therapeutic implications. **Recent findings:** Eosinophilic bronchiectasis is identified using blood or sputum eosinophilia, although optimal biomarkers and thresholds are unclear. Eosinophilic inflammation occurs in bronchiectasis and overlapping disease states characterized by type 2 immune activation, including asthma, allergic bronchopulmonary aspergillosis, and cystic fibrosis, complicating diagnosis but potentially informing disease severity, prognosis, and treatment options. Evidence for targeted therapies is limited to largely observational data. Patients with elevated eosinophil counts may derive greater benefit from inhaled corticosteroids. Early studies of biologics appear promising, particularly in patients with coexisting ABPA or asthma, although their benefit in eosinophilic bronchiectasis independent of these conditions remains uncertain. **Summary:** Eosinophilic bronchiectasis represents a distinct inflammatory endotype that may enable a more personalized approach to disease management. Future studies should identify clinically relevant biomarkers to standardize its definition, clarify its relationship with overlapping type 2 airway diseases, and guide the use of inhaled corticosteroids and biologics. **Keywords:** biologic therapy; bronchiectasis; eosinophils; inhaled corticosteroids; type 2 inflammation.
