## Executive GIST (TL;DR)
- Community-based drug checking collected anonymous drug samples in Southern California between 2023 and 2026; samples were screened using DART-MS and subsets quantified by LC-MS/MS with some enantiomeric analysis. - Investigators reported the first detection of **medetomidine** in May 2025 and a rise in co-detection with **fentanyl** from ≈6% to ≈25% by August 2026, calculated on a quarterly basis. - The study team observed an average **medetomidine** mass fraction of 0.4%; the source did not report specific dose ranges or exact sample counts for the full dataset in the abstract. - Common co-detected substances included **fentanyl**, **xylazine**, and adulterants such as lidocaine; co-occurrence patterns were tabulated by the authors. - All 13 samples that underwent enantiomeric analysis were racemic, indicating the material likely did not originate from diverted human pharmaceutical products, the authors noted. - Data availability: individual line-level data are unavailable; aggregated data are posted at drugcheckinglosangeles.com/viz, per the authors. - The manuscript is a preprint and has not been peer reviewed; investigators and readers should treat findings as preliminary and not guidance for clinical care. - Limitations reported or implied by the authors include lack of individual-level data access and absence of reported clinical outcome linkage within this analysis.

Multiple states have passed legislation to control xylazine, a pharmacologically similar sedative in the illicit opioid supply. Medetomidine may function as a highly potent substitute for xylazine with significant overdose risk and withdrawal potential. Medetomidine has previously been described mostly in eastern U.S. fentanyl markets. Here, we characterize medetomidine-containing drug samples obtained in Southern California 2023-2026. Methods: Anonymous participants in Southern California voluntarily provided drug products to a community-based drug checking program. All samples underwent direct-analysis-in-real-time mass spectrometry (DART-MS). A subset was quantified by liquid-chromatography tandem mass spectrometry (LC-MS/MS). A smaller subset underwent enantiomeric analysis. We calculated other compounds co-present with medetomidine, the prevalence of medetomidine in fentanyl over time, the concentration of medetomidine, and the enantiomer makeup of medetomidine. Results: The quarterly prevalence of medetomidine co-present with fentanyl rose from ≈6 % in May 2025 (month of first detection) to ≈25 % by August 2026. The average medetomidine concentration was 0.4 % mass fraction. The most common co-detected substances with medetomidine were fentanyl and adulterants such as lidocaine and xylazine. All medetomidine samples that underwent enantiomeric analysis (n = 13) were identified as racemic. Conclusion: Medetomidine represents a rapidly emerging adulterant in the Southern California fentanyl supply. All samples assessed for enantiomer status were racemic medetomidine, suggesting they were not diverted from human pharmaceutical or medical supplies. Given its high potency, a small amount ( ## Funder Information Declared Centers for Disease Control and Prevention, https://ror.org/042twtr12, NH28CE003543-01-00 National Institutes of Health, R01 DA057630 National Center for Advancing Translational Sciences, TL1TR001883 National Center for Injury Prevention and Control James B.
