## Executive GIST (TL;DR)
- This retrospective single-centre observational study compared **robotic** and **laparoscopic** ileal pouch-anal anastomosis (**IPAA**) for medically refractory **ulcerative colitis** at Sant'Orsola University Hospital, Bologna. - Consecutive minimally invasive proctectomies with IPAA and diverting loop ileostomy performed between 2017 and 2025 were reviewed. - Investigators analysed 169 patients in total: 131 in the laparoscopic cohort and 38 in the robotic cohort. Demographics were reported as comparable between groups. - A propensity score-matched analysis was used to compare demographic, clinical, surgical and postoperative data. - The robotic group showed a lower conversion rate (2.6% robotic vs. 13.0% laparoscopic), a difference that did not reach statistical significance (p = 0.078). - Time to return to oral diet favoured the robotic cohort with statistical significance (p < 0.001). - Length of stay, operative duration and overall postoperative complication rates were reported as comparable across approaches. - Severe complications (Clavien-Dindo ≥ III) occurred in 7% of the laparoscopic cohort; the robotic cohort did not exceed Grade II complications. - No robotic cases required 30-day reoperation; the laparoscopic cohort had a 30-day reoperation rate of 2.3%. - Authors concluded robotic IPAA produced comparable results to laparoscopy for most endpoints, with trends toward fewer conversions, lower complication severity and faster oral intake recovery. - The source notes that further comparative evidence is required; the study is limited by its retrospective, single-centre design and the period of case collection (2017–2025).

While laparoscopy is a well-established alternative to open surgery, further comparative evidence between laparoscopy and robotic approaches is required. This study aims to evaluate intraoperative performance and short-term postoperative outcomes in laparoscopic and robotic IPAA in a high-volume referral centre. This retrospective single-centre study evaluated consecutive patients undergoing minimally invasive proctectomy with IPAA and diverting loop ileostomy between 2017 and 2025 at Sant' Orsola University Hospital, Bologna. Demographic, clinical, surgical, and postoperative data were compared between the laparoscopic and robotic cohorts using a propensity score-matched analysis. This study included 169 patients: 131 in the laparoscopic group and 38 in the robotic one. Demographic characteristics were comparable. The robotic cohort presented a lower conversion rate, though this did not reach statistical significance (13.0% vs. 2.6%, p = 0.078). On the other hand, it presented a faster return to oral diet (p - [DOI](https://doi.org/10.1136/bmj.2.6130.85) - [PubMed](https://pubmed.ncbi.nlm.nih.gov/667572/) - [PMC](https://pmc.ncbi.nlm.nih.gov/articles/1605901/) 2. 1.
