## Executive GIST (TL;DR)
- The source is a preprint titled **Epigenome-Wide Association Study of the Upper-Airway Microbiome Involved in Asthma Exacerbations**, posted on medRxiv with DOI https://doi.org/10.64898/2026.10.02.26364005. - The manuscript lists a large, multi-institutional author group from universities and hospitals across Spain, the USA, the Netherlands, Germany, Slovenia and other countries, indicating an international collaboration. - Lead affiliations include the Genomics and Health Group, Universidad de La Laguna, and multiple pediatric, hospital, and public health centers; many authors have ORCID identifiers reported. - The article is explicitly labeled a **preprint** and has not been certified by peer review; medRxiv warns it should not be used to guide clinical practice. - The page content provided to the clinical editor includes title, author names, affiliations, and the preprint disclaimer, but the excerpt does not include the study abstract, methods, results, statistics, sample size, cohort characteristics, specific epigenetic loci, microbial taxa, or conclusions. - Because the source excerpt omits study details, no assumptions or additional clinical interpretations about associations, effect sizes, or clinical implications can be drawn from this text alone. - Any clinical or translational implications, mechanistic inferences, or recommendations for practice are not reported in the provided source material; readers should consult the full preprint for complete data and await peer review. - The available metadata documents multidisciplinary participation (genomics, pediatrics, respiratory medicine, public health), suggesting the study aims to link **epigenetic variation** and the **upper-airway microbiome** with **asthma exacerbations**, but specific findings and methods were not reported in the supplied content. - For clinicians and researchers, the key actionable point from this excerpt is the existence of the preprint and its DOI; further evaluation requires access to the full manuscript and peer-reviewed publication. - The curated summary preserves the original non-peer-reviewed status and refrains from inferring results absent from the source document.

Objectives : To identify whole-blood DNAm markers associated with the upper-airway microbiome related to asthma exacerbations despite ICS treatment. Methods : We analyzed salivary, nasal, and pharyngeal microbiome in 242 asthma patients (GEMAS) for discovery. We tested associations between whole-blood DNAm at 703,505 CpGs and 21 exacerbation-related microbiome traits. Replication for the nasal microbiome was evaluated in 272 (Project Viva) and 66 (SysPharmPediA) children with and without asthma. We analyzed differentially methylated regions (DMRs), gene enrichment, associations with blood gene expression (RNA-seq, n=119), and DNAm predictors of plasma proteins. Measurements and Main Results : Whole-blood DNAm at 73 CpGs and 550 DMRs was associated with 17 exacerbation-related microbiome traits (false discovery rate [FDR]<0.05), including genes implicated in immune and inflammatory responses and the 17q12-q21 asthma locus ORMDL3 . Most CpGs were associated with Campylobacter (pharyngeal) and Leptotrichia (nasal) abundance. Ten DNAm markers were replicated ( p <0.05), including the corticosteroid insensitivity-related gene PTPRR associated with nasal bacterial richness. Enrichment analyses identified genes related to diabetes, obesity, smoking, TGF-β, and Wnt signaling and regulated by trichostatin A, NF-κB, and the glucocorticoid receptor (FDR<0.05). The CpG cg18994033, associated with pharyngeal Tannerella , was associated with ITM2C whole-blood expression. Nine DNAm-predicted plasma proteins involved in inflammation, tissue remodeling, and TGF-β signaling were associated with seven exacerbation-related microbiome traits, including Streptococcus and Selenomonas . Conclusions : Whole-blood DNAm markers implicated in inflammation, immunity, and asthma comorbidities are associated with the upper-airway microbiome linked to asthma exacerbations.
