## Executive GIST (TL;DR)
- This preprint describes a clinically implemented **MR-simulation** protocol tailored for head and neck radiation oncology targeting the **skull base**, **oral cavity**, and **larynx/pharynx**. - Protocol development involved a multidisciplinary team: radiation oncologists, medical physicists, radiation therapists, and neuroradiologists. - Imaging was performed on both 1.5T and 3T MR-simulation scanners in a cohort of 41 patients with head and neck cancer and 3 healthy volunteers. - The protocol set includes multiple sequence families: **Dixon**-based methods, **SPAIR**-based fat suppression approaches, **short T2** sequences, and **quantitative imaging** methods. - The manuscript outlines practical steps to optimize these sequences for clinical implementation within a radiation oncology clinic, emphasizing reproducibility and workflow integration. - All participants were prospectively enrolled with institutional review board approval and written informed consent; IRB protocol numbers for volunteers and patients are reported. - Data produced in the study are contained within the manuscript; supplementary material and data/code links are provided in the source record. - Funding sources include multiple federal grants, institutional fellowships, and industry partners; one author disclosed unrelated financial relationships with several vendors. - The authors note this work is a preprint that has not undergone peer review and should not be used to guide clinical practice without further validation. - Practical aims include improved visualization for target delineation and streamlined MR-simulation implementation at radiation oncology centers by sharing optimized sequence parameters and workflow considerations.

Through collaboration with medical physicists, radiation therapists, radiation oncologists, and neuroradiologists, our group clinically implemented a standard set of magnetic resonance imaging (MRI) sequences on an MR-simulation scanner for three anatomical regions: skull base, oral cavity, and larynx/pharynx. A total of 41 patients with head and neck cancer and 3 volunteers were scanned on either a 1.5T or 3T MR-simulation scanner. Sequences described include Dixon-based approaches, SPAIR-based approaches, short T2-based approaches, and quantitative imaging methods. We describe and outline the steps to optimize these notable MRI sequences to aid in successful implementation in a radiation oncology clinic. ### Competing Interest Statement CDF has received unrelated travel/honoraria/speaking fees from Elekta AB, Philips Medical Systems, Siemens Healthineers/Varian, and GE Healthcare.
