# RG-SSL-LOC: Report-Guided Semi-Supervised Model Improves Prostate Cancer Detection on Biparametric

## Executive GIST (TL;DR)
- A multicenter study developed and validated a **report-guided semi-supervised lesion-only model (RG-SSL-LOC)** for scalable detection of clinically significant prostate cancer on **biparametric MRI (bpMRI)**. - Model development and validation used 13,706 bpMRI examinations from 13,630 patients across 27 centers. - Three segmentation approaches were compared: fully supervised learning (FSL), a prior report-guided semi-supervised method (RG-SSL), and the proposed **RG-SSL-LOC** lesion-only teacher model. - At the lesion level, RG-SSL-LOC produced higher median **Dice** (0.49) than FSL (0.41) and RG-SSL (0.40); both comparisons reached p<.001. - At case level, RG-SSL-LOC achieved **AUC**s of 0.83 (external retrospective), 0.82 (external prospective), and 0.87 (internal prospective). - Case-level AUC comparisons versus FSL and RG-SSL reported variable p-values across cohorts; some comparisons reached statistical significance (for example, versus FSL in the external prospective and internal prospective cohorts). - When combined with clinico-radiologic variables in a multimodal model, RG-SSL-LOC significantly improved AUC compared with clinico-radiologic variables alone: 0.85 vs 0.80 (external retrospective, p=.002), 0.87 vs 0.84 (external prospective, p=.008), and 0.91 vs 0.88 (internal prospective, p<.001). - In the internal prospective cohort the multimodal integration reduced unnecessary biopsies by 15.19%. - The study reports multicenter prospective performance and claims improved segmentation quality and multimodal detection, while noting this is a preprint not yet peer reviewed. - Data sources include publicly available and consortium datasets (PI-CAI, UCLA, PROMIS on request, ProstateNET within ProCAncer-I) and ethics approvals and consent/waivers were obtained at participating sites. - Funding support was declared from the European Union Horizon 2020 program (grant 952159).

Materials and Methods: A multicenter dataset of 13,706 bpMRI examinations (13,630 patients, 27 centers) was used for model development/validation. Three segmentation models (fully supervised learning [FSL], a state-of-the-art report-guided semi-supervised approach [RG-SSL], and the proposed RG-SSL-LOC) were evaluated at lesion- and case-level on external retrospective, external prospective, and internal prospective cohorts. Predictions from the best-performing model were combined with clinico-radiologic variables in a multimodal approach. All case-level results were compared with PI-RADS.
