## Executive GIST (TL;DR)
- The authors describe a single-center, prospective 1:1 randomized, non-inferiority trial comparing **single-port** and **multiport** extraperitoneal robotic radical prostatectomy using the domestic **KangDuo** robotic system. - Eligible subjects are men with localized prostate cancer staged cT1c–cT2cN0M0; the protocol randomizes patients to experimental (single-port) or control (multiport) arms. - The trial uses standardized enhanced recovery after surgery (ERAS) protocols for all participants; perioperative care is harmonized across groups. - The prespecified primary endpoint is postoperative **positive surgical margin** rate; secondary assessments include perioperative parameters, postoperative recovery, medium-term urinary and sexual functional recovery, and oncological outcomes. - Investigators hypothesize non-inferior oncological radical effect for single-port surgery, with potential minimally invasive benefits and comparable safety and functional recovery to multiport surgery. - Trial registration is listed: Chinese Clinical Trial Registry ChiCTR2600125669. - Ethical oversight: Ethics Committee of Lishui Central Hospital approved the study (Approval No. provided); written informed consent will be obtained. - The article is a medRxiv preprint and has not undergone peer review; the source did not report sample size, enrollment timelines, or detailed statistical assumptions in the abstract. - Data availability: no datasets generated yet; the authors state data will be made available on reasonable request after study completion. - The report does not provide interim results, safety event rates, or concrete timelines for follow-up; those details are to be determined during the trial execution.

Conventional multiport robotic surgery is widely applied yet associated with more abdominal trauma and postoperative discomfort. Single-port robotic surgery can further minimize surgical injury. The domestic KangDuo robotic system has been gradually used in urological oncology, but high-level prospective evidence comparing single-port and multiport extraperitoneal radical prostatectomy remains scarce. This trial aims to compare the efficacy and safety of these two surgical approaches. Methods: This is a single-center, prospective, 1:1 randomized, non-inferiority trial. Eligible patients diagnosed with cT1c–cT2cN0M0 localized prostate cancer will be randomized to receive either single-port (experimental group) or multiport (control group) extraperitoneal robotic radical prostatectomy using the KangDuo system. All participants will receive standardized enhanced recovery after surgery (ERAS) protocols. The primary endpoint is postoperative positive surgical margin rate. Perioperative parameters, postoperative recovery, medium-term functional recovery and oncological outcomes will be assessed comprehensively. Expected results: We hypothesize that the single-port approach is non-inferior to the multiport approach in terms of oncological radical effect. The single-port technique is expected to provide better minimally invasive benefits, with comparable perioperative safety, urinary/sexual function recovery and mid-term oncological outcomes between groups. Conclusions: This randomized trial will provide high-quality clinical evidence for the application of domestic single-port robotic surgery. The findings will facilitate individualized surgical selection for patients with localized prostate cancer. Trial Registration: Chinese Clinical Trial Registry, ChiCTR2600125669 (http://www.chictr.org.cn/showproj.aspx?proj=2600125669). ### Competing Interest Statement The authors have declared that no competing interests exist. ### Clinical Trial ChiCTR2600125669 ### Author Declarations I confirm all relevant ethical guidelines have been followed, and any necessary IRB and/or ethics committee approvals have been obtained.
