This tribute is written in memory of Professor Paul Mitrofanoff (Fig. 1) and in honour of his exceptional accomplishments and lasting contributions to urology.
His name is known worldwide, primarily through the procedure that bears it to create a continent, catheterisable channel (appendicovesicostomy) that restores autonomy, dignity, and social participation to children and adults who are unable to catheterise through the urethra. Some 50 yr after the first patient underwent the procedure, this principle remains clinically indispensable and continues to evolve and is regularly revisited in contemporary series and refined via modern reconstruction strategies, including minimally invasive and robotic approaches, which confirms a legacy that still shapes practice, education, and patient pathways.
