We read with great interest the article by Mannucci et al1 on the potential value of a next-generation cell-free and exosome-bound microRNA blood test as a complementary noninvasive screening strategy for early-onset colorectal cancer (EOCRC). The reported liquid biopsy performance is impressive, showing 97.3% sensitivity for stage I-III EOCRC and, notably, 61.5% for premalignant lesions with high-grade dysplasia, much higher than other reports.2,3 Remarkably, the rapid clearance of biomarker levels after surgery reinforces their tumor specificity.
