Using 2 large, population-based cohorts with available phosphatidylethanol (PEth) measurements, we developed and validated the MetALD-ALD Prediction Index (MAPI), a novel, accurate, and scalable indirect alcohol biomarker panel for distinguishing individuals with MetALD-ALD from metabolic dysfunction-associated steatotic liver disease. MAPI may guide confirmatory PEth testing in clinical practice and be applied in observational cohort studies or clinical trials lacking PEth data.
