Diabetic gastroparesis (DGP) is a severe complication of diabetes that can lead to disabling gastrointestinal discomfort, weight loss, malnutrition, and increased mortality.1–4 Until now, all treatment options for DGP have proved disappointing because of their poor efficacy. A promising treatment for DGP is the minimally invasive endoscopic gastric pyloromyotomy (G-POEM).5 To assess the efficacy of G-POEM for DGP we performed a randomized, double-blinded, sham-controlled trial designed to demonstrate the effects on gastric emptying, gastrointestinal symptoms, pyloric function, and blood glucose control (Supplementary Material).
