We read with great interest the prospective cohort study by Chari et al,1 titled “Risk of Pancreatic Cancer in Glycemically Defined New-Onset Diabetes: A Prospective Cohort Study.” The study confirms a significantly elevated 3-year risk of pancreatic cancer following glycemically defined new-onset diabetes (GNOD) through an electronic health record surveillance algorithm. This represents a significant advance in early detection of pancreatic cancer. However, a profound internal contradiction within the study data compels us to re-examine the fundamental premise of GNOD as a window for early detection.
