This retrospective study describes trends in cataract surgical volume, district‑level surgical burden, and early postoperative visual outcomes among patients treated through a multi‑district outreach eye‑camp programme run by Bashundhara Eye Hospital and Research Institute in Bangladesh. Data covered surgeries performed across eight districts from 2016 through 2025; 2021 was excluded because of the COVID‑19 pandemic. The purpose was to quantify temporal changes in surgical activity and to identify factors associated with early (day‑1) visual outcomes.
The analysis used routine programme records of cataract surgeries performed during outreach camps. Annual surgical volume trends were modeled using Poisson regression to estimate incidence rate ratios (IRRs) for year‑to‑year changes. Postoperative visual outcome on day 1 was categorized according to World Health Organization (WHO) criteria into good, borderline, or poor. Predictors of worse day‑1 outcome were assessed using univariable and multivariable ordinal logistic regression. The available dataset comprised 1,929 records; incomplete records were excluded. The IRB of Bashundhara Eye Hospital and Research Institute waived formal ethical approval. The authors report that data are available on reasonable request.
Surgical activity through the outreach programme increased substantially over the study period. Total included records numbered 1,929. Annual volume rose from 34 surgeries in 2016 to a peak of 677 surgeries in 2023. Poisson regression quantified the upward trend with an IRR of 1.20 (95% CI: 1.180–1.220; p < 0.001), indicating an average 20% increase in surgical counts per year over the observed interval.
The dominant surgical technique recorded was small incision cataract surgery (SICS), accounting for 99.43% of cases in the dataset. No other specific technique proportions or intraoperative complication rates were reported in the source.
Postoperative day‑1 visual outcomes were classified per WHO standards. Overall, 70.09% of eyes achieved a good visual outcome on day 1, 19.44% were categorized as borderline, and 10.47% as poor. The report provides these proportions as the primary early outcome distribution but does not report refracted visual acuity values, longitudinal outcomes beyond day 1, or standardized refraction results.
Regression analyses identified patient age and district of surgery as independent predictors of worse day‑1 outcome. Increasing age was associated with higher odds of a worse category on day 1: patients older than 70 years had approximately two to three times greater odds of a worse outcome compared with younger patients. District‑level differences persisted after adjustment: the districts of Chapainawabganj and Kushtia had lower adjusted odds of worse day‑1 outcome relative to Brahmanbaria. Sex was associated with outcome in univariable (unadjusted) analysis but lost significance in multivariable models, indicating that sex was not an independent predictor after accounting for other factors included in the model.
The authors report several limitations affecting interpretation and generalizability of findings:
These constraints limit conclusions about durable visual rehabilitation and the full set of determinants of outcome after outreach cataract surgery.
Across eight districts, cataract surgical volume delivered through the outreach programme increased markedly between 2016 and 2025, with a peak in 2023. On day 1 after surgery, about seven in ten eyes achieved a good outcome by WHO criteria. Older age and district of surgery were the main independent predictors of worse early outcome; sex was not an independent predictor after adjustment. The findings highlight substantial increases in outreach surgical capacity and reveal geographic and age‑related disparities in early outcomes. Programmes planning cataract outreach services may consider targeted quality‑improvement and follow‑up strategies for older patients and exploration of district‑level practice or resource differences to address outcome variation.
Note: The source document did not report longitudinal outcomes beyond day 1, details on intraoperative or postoperative complications, standardized refractive data, or the full set of covariates included in multivariable models; these details were therefore not available for inclusion here.
