Excessive alcohol consumption is a primary contributor to preventable deaths in the United States, leading to over 178,000 deaths annually and imposing a significant economic burden exceeding $200 billion. Approximately 10% of adults in the U.S. are diagnosed with alcohol use disorder (AUD), yet only a small fraction—only about 2%—receive medication-assisted treatment. Current FDA-approved treatments for AUD, while effective, face challenges related to adherence and tolerability, limiting their effectiveness in practice.
GLP-1 receptor agonists (GLP-1 RAs) are mostly recognized for their applications in managing type 2 diabetes and obesity. Emerging observational data suggest potential benefits of these medications in reducing alcohol-related adverse outcomes, indicating a need for further clarification of their effects in AUD populations. Despite indications from observational studies, randomized trials remain scarce and their outcomes inconsistent. Recent trials have indicated that GLP-1 RAs may influence drinking behaviors, yet definitive conclusions are elusive, warranting more focused investigation.
This study employed a retrospective cohort design utilizing health record data from various U.S. healthcare systems spanning the years 2018 to 2024. Utilizing a framework of target trial emulation, the research compared alcohol-related hospitalizations among adults initiating newer GLP-1 RAs—namely, semaglutide and tirzepatide—against relevant active comparator medications. The study organized participants into four distinct trials based on clinical context and indication.
The study included adults diagnosed with both AUD and either type 2 diabetes or obesity who commenced treatment with the newer GLP-1 RAs or active comparators during the specified period. Participants were categorized into four target trials: anti-diabetic medication (ADM), anti-obesity medication (AOM), medications for alcohol use disorder with T2D (MAUD-T2D), and medications for alcohol use disorder with obesity (MAUD-obesity).
By refining the participant cohorts based on specific health conditions, the study aimed to enhance the relevance of findings across varied clinical settings, assessing how GLP-1 RAs affect alcohol-related outcomes and hospitalizations in these populations.
The primary outcome assessed in this study was the time to first alcohol-related hospitalization or emergency department visit within one year following treatment initiation. Negative control outcomes, specifically non-alcohol-related hospitalizations, were also analyzed to validate associations and control for confounding factors. The researchers utilized Cox proportional hazards models to analyze these outcomes while employing propensity score matching for robust adjustment of confounding variables.
The analysis employed various statistical methodologies to ensure the validity of findings. Population balancing techniques were utilized to mitigate selection bias due to non-random treatment allocation. Moreover, the study accounted for informative censoring to ensure that the population's characteristics were consistent across different treatment groups.
Statistical significance was assessed through hazard ratios, estimating the risk of alcohol-related hospitalizations between the GLP-1 RA cohort and comparators over the follow-up period.
In total, 40,703 adults met the eligibility criteria across the four target trials. The findings indicated that initiating treatment with newer GLP-1 RAs was significantly associated with lower rates of alcohol-related hospitalizations. Specifically, the hazard ratios for alcohol-related admissions were found to be 0.74 for the ADM trial, 0.68 for the AOM trial, 0.37 for the MAUD-T2D trial, and 0.35 for the MAUD-obesity trial when compared with respective medication alternatives.
These results suggest a potentially protective effect of GLP-1 RAs against hospitalizations due to alcohol-related issues in targeted populations.
The initiation of newer GLP-1 RAs among adults diagnosed with alcohol use disorder and either type 2 diabetes or obesity is associated with a reduced risk of alcohol-related hospitalizations. This study's results contribute important insights regarding the potential role of GLP-1 RAs in managing AUD and highlight opportunities for integrating these medications into treatment strategies for affected individuals. Further research is essential to elucidate the mechanisms at play and to substantiate these findings in broader contexts.
