We read with interest and concern the article by Engelmann et al1 proposing a new definition of acute-on-chronic liver failure (ACLF). ACLF is an important concept in patients with cirrhosis and represents a cause of death for many. The most widely used ACLF definition was originally conceived from the CANONIC2 cohort and has led to considerable global interest in the definition of this syndrome and intense but, so far unsuccessful, attempts at developing effective therapeutics beyond liver transplantation3.
