I read with great interest the important study by Murphy et al.[1], which confirmed that the use of invasive intracranial pressure monitoring (IICPM) in patients with acute liver failure (ALF) has significantly declined without negatively impacting key patient outcomes such as survival through a retrospective cohort analysis. More importantly, the significance of this study extends beyond validating a shift in clinical practice. It highlights a three-fold paradigm evolution in neuroprotection for ALF, prompting us to reconsider future directions.
