We read with great interest the report by Zhang et al. describing the first genetically engineered pig-to-human auxiliary liver xenotransplantation.1 This landmark case offers rare in vivo insights into xenogeneic hepatic physiology. We wish to highlight several conceptual issues that may guide future clinical development.
