We read with great interest the article by Zhang et al.1 published in the Journal of Hepatology, reporting the first pig-to-human liver xenotransplantation in a patient. Undoubtedly, this study represents a landmark breakthrough in the field of liver transplantation.1,2 However, we would like to raise several critical concerns for the authors. Clarifying these issues could significantly enhance the understanding of the immunological challenges inherent in clinical xenotransplantation.
