A 38-year-old man evaluated for elevated transaminases had incidental renal cysts on ultrasonography: multiple cysts in an enlarged left kidney and a normal-sized right kidney with isolated cysts. He reported no flank pain or hematuria, except after exertion.
Blood pressure and renal function were normal. Contrast-enhanced computed tomography (Figure 1) confirmed asymmetric cystic disease, with left and right kidneys volumes of 1681 and 220 cm3 (Supplementary Figure S1), respectively, and no liver involvement.
