A 43-year-old woman with a 3-year history of microscopic hematuria and low-grade proteinuria, but without extrarenal manifestations, was evaluated for increasing protein excretion. Her 24-hour urine protein was 615 mg/24 h, largely accounted for by free κ light chain (545 mg/24 h), with only minimal albuminuria (17.2 mg/24 h).
Serum studies revealed monoclonal IgA-κ by immunofixation electrophoresis, and κ-restricted light chain in serum and urine. Serum κ/λ ratio was 2.97, and the urinary κ/λ ratio was 27.53.
