A 75-year-old woman with recurrent urinary tract infections was prescribed Uribel (methenamine 118 mg, sodium phosphate monobasic 40.8 mg, phenyl salicylate 36 mg, methylene blue 10 mg, hyoscyamine sulfate 0.12 mg [Star Pharmaceuticals]) 118 mg 3 times daily for symptom relief. She presented to the hospital after a mechanical fall, where her urine appeared distinctly green (Figure 1).
On examination, she was hemodynamically stable. Laboratory studies showed normal liver function and bilirubin levels, ruling out obstructive jaundice.
