Virtual reality (VR) has rapidly evolved from an experimental technology into a practical tool for neurologic and neuro-ophthalmological assessment and rehabilitation. Modern head-mounted systems with integrated motion and eye tracking can capture detailed, quantitative biomarkers of gait, balance, visuospatial function, eye movements, and visual fields while delivering controlled, engaging environments. In neuro-ophthalmology, VR-based perimetry and oculomotor testing increasingly demonstrate acceptable agreement with gold-standard methods and may expand access to visual field and eye movement assessment. Across indications, careful psychometric validation, attention to safety, data privacy, and workflow integration are essential before routine clinical use.
