A 69-year-old woman was referred to our department after monoclonal gammopathy was detected during routine testing. Her medical history included dyslipidaemia, hypothyroidism, and right hip replacement due to congenital dysplasia.
Physical examination, complete blood count, and serum biochemical tests were unremarkable. Immunofixation revealed monoclonal IgGλ (total IgG 1060 mg/dL), with a normal free light chain (FLC) ratio.
Urine immunofixation was negative. A bone marrow biopsy showed 12% clonal plasma cell infiltration, consistent with smouldering multiple myeloma (SMM) pending whole-body, low-dose CT (WBLD–CT).
