The care of immunocompromised adults with acute respiratory failure (ARF) remains one of the greatest challenges of intensive care medicine.1 Management of these patients is challenging because diagnostic, therapeutic, and supportive care decisions must be made rapidly in the setting of aetiological uncertainty, atypical presentations, and high risk of deterioration. In this population, the differential diagnosis is broad, ranging from opportunistic infections and drug toxicity to inflammatory or malignant processes.
