A 15-year-old male adolescent was referred to pediatric nephrology for evaluation of several months of hypertension detected during a routine physical examination. He was asymptomatic and had no history of kidney disease, urinary tract infection, or prematurity.
On presentation, his blood pressure measured 148/84 mm Hg, and physical examination was otherwise unremarkable. Laboratory evaluation revealed normal serum creatinine, electrolytes, and urinalysis without hematuria or proteinuria.
Kidney ultrasonography showed moderate to severe right hydronephrosis with preserved cortical thickness (Supplementary Figure S1) and no mass or stones.
