Eli Lilly's oral medication orforglipron has shown superior glycemic control in Phase 3 trials compared to existing therapies, which could transform diabetes management.
Eli Lilly has successfully cleared a significant clinical milestone with its experimental oral medication, orforglipron, often referred to as a "Ozempic-in-a-pill". This once-daily oral treatment has met all primary and secondary endpoints in two crucial Phase 3 trials, known as ACHIEVE-2 and ACHIEVE-5. The gathered data indicates that orforglipron provides superior glycemic control when compared to a standard SGLT-2 inhibitor as well as a placebo. If it gains approval, orforglipron could revolutionize diabetes management by combining the efficacy of injectable treatments with the convenience of a daily tablet.
Findings made public in October 2025 reveal that orforglipron significantly lowered A1C levels — which represent the average blood sugar over a period — alongside body weight reductions over a duration of 40 weeks. Distinct from existing oral GLP-1 medications, orforglipron is classified as a non-peptide and small-molecule drug. This chemical structure is particularly noteworthy; while traditional peptide drugs are often broken down in the digestive system, this non-peptide alternative retains its integrity and is effectively absorbed when taken in pill form, free from the strict dietary constraints often required for its predecessors.
As part of the GLP-1 (glucagon-like peptide-1) receptor agonist category, orforglipron mimics a gut hormone that encourages insulin secretion and mitigates digestion. In contrast to SGLT-2 inhibitors, which function via the kidneys by aiding in the expulsion of excess sugar through urine, Lilly’s latest evidence suggests that orforglipron’s hormonal mechanism yields superior reductions in blood sugar levels.
The ACHIEVE development program is assessing orforglipron through five global registration trials. Previous studies had shown that orforglipron performed better than oral semaglutide, but the conclusive results from the ACHIEVE-2 and ACHIEVE-5 trials now reinforce its superiority against existing treatment standards.
In terms of cardiovascular risk factors, orforglipron has also delivered notable improvements. The safety profile aligns with other medications in the GLP-1 class; mild to moderate gastrointestinal events surfaced as the most frequently reported side effects, without any noted liver safety issues.
Analyzing the ACHIEVE-2 trial, which made a direct comparison between orforglipron and the SGLT-2 inhibitor dapagliflozin, patients who were inadequately controlled while on metformin and received a 36 mg dose of orforglipron experienced an A1C reduction of 1.7%. This was more than double the 0.8% reduction observed within the SGLT-2 cohort.
Jeff Emmick, the senior vice president of product development for Lilly's Cardiometabolic Health division, remarked, "Orforglipron has now demonstrated superiority over two active comparators," underscoring its potential to become a new standard in diabetes management.
The ACHIEVE-5 trial assessed a more complex demographic — adults using titrated insulin glargine. This trial maintained constant adjustments to the glargine dosage to ensure stable fasting blood sugar levels. Notably, this trial exhibited that presenting orforglipron alongside insulin glargine led to a 2.1% reduction in A1C levels, contrasting with the 0.8% reduction found in the placebo group. This is particularly crucial because many patients find that insulin therapy alone fails to meet their health objectives or results in undesired weight gain. The addition of orforglipron facilitated a more aggressive blood sugar management while concurrently encouraging weight loss.
In summary, orforglipron has showcased substantial improvements in glycemic control without significant safety concerns, maintaining a profile consistent with other GLP-1 medications. Looking to the future, Lilly intends to file for regulatory approval for orforglipron in 2026.
