Universal Health Services (UHS) has reported a robust second quarter for 2026, surpassing expectations. The company announced a net income of $358.4 million, or $5.98 per diluted share, exceeding Wall Street estimates and increasing from $353.2 million, or $5.43 per diluted share, from the prior year. Net revenues also came in strong at $4.64 billion, marking an 8.3% year-over-year increase.
However, UHS executives indicated that full-year projections for earnings have been revised downwards. Despite raising revenue guidance, the expected performance of individual facilities and rising professional and general liability expenses have prompted caution concerning earnings per share (EPS). CFO Steve Filton noted that the company’s adjusted EPS guidance has now adjusted to a range between $22.28 and $23.65, previously $22.53 to $24.52.
The new revenue guidance indicates a net revenue forecast between $18.50 billion and $18.76 billion, down from the previous estimate of $18.42 billion to $18.79 billion. Adjusted EBITDA net of non-controlling interests is now projected between $2.61 billion and $2.72 billion, again lower than the prior range of $2.64 billion to $2.79 billion. This revision reflects a slow start to the year, despite the positive trajectory noted in demand across acute and behavioral healthcare services.
UHS has attributed some of its revenue fluctuations to fluctuations in Medicaid supplemental funding, which has provided a net benefit of approximately $150 million. This funding includes previously unaccounted amounts from states like Florida and Texas, significantly improving their financial outlook for the year.
In their earnings call, UHS leadership expressed optimism regarding a recovery in healthcare demand. The reopening of facilities, including a 2.5% increase in acute care beds, is expected to contribute to improved performance in the second half of the year. This highlights UHS's anticipation of recovery trends, particularly in the Nevada market that experienced softness last year.
Filton pointed out that while same-facility acute care admissions rose by 2.9%, expectations were lowered for both acute and behavioral health adjusted admissions. The adjusted admissions for same-facility behavioral health services saw a modest increase of 0.5%, with total net revenues also reflecting growth of 7.4% in this sector.
Concerns tied to the Affordable Care Act (ACA) health insurance exchange markets have also prompted revisions in UHS's forecasts. The company increased its projected pre-tax impact from these exchanges by $10 million, now estimating a total impact of $85 million. Adjusted admissions among exchange patients fell by approximately 15% year-over-year in the second quarter, raising concerns that a larger number of patients who lost coverage have remained uninsured rather than replacing it with commercial plans.
Filton noted that the assumption that a portion of those losing coverage would transition to employer-based insurance did not hold true, thus adjusting their financial strategies accordingly.
Looking forward, UHS plans to address operational challenges including rising professional fees, which have been influenced by inflation. The company has incorporated a projected annual inflation rate of 7% to 9% into their guidance, reflecting the pressures felt across the industry. UHS is actively responding by hiring hospital-based physicians and reassessing contracts to manage these increases effectively.
On the behavioral health side, UHS is progressing with its acquisition of virtual behavioral care platform Talkspace, expected to close in mid-August. This strategic move aims to leverage referrals and enhance hybrid care capacity, further expanding UHS's service offerings in the behavioral sector.
UHS's share repurchases also saw a significant rise to $320 million this quarter compared to $127 million in the previous quarter, reflecting confidence in the company’s financial strategy following the strong Q2 performance.
